400;\">Dalam beberapa dekade terakhir, kemajuan dalam biologi molekuler dan genomik telah membuka era baru dalam dunia medis: <\/span>precision medicine<span style=\"font-weight: 400;\">. Pendekatan ini bertujuan untuk menyesuaikan diagnosis dan terapi berdasarkan profil genetik, lingkungan, dan gaya hidup individu, menggantikan paradigma &#8220;satu obat untuk semua&#8221; yang selama ini mendominasi praktik klinis (National Research Council, 2011). Artikel ini akan mengulas bagaimana precision medicine merevolusi diagnostik, memanfaatkan teknologi terkini untuk meningkatkan akurasi dan personalisasi perawatan kesehatan.<\/span><\/p>\n<p><b>Genomik sebagai Fondasi<\/b><b><br \/>\n<\/b><span style=\"font-weight: 400;\">Kemampuan untuk melakukan sekuensing genom manusia (whole-genome sequencing) menjadi katalis utama precision medicine. Proyek Genom Manusia (Human Genome Project) yang selesai pada 2003 membuka jalan bagi identifikasi variasi genetik yang berkorelasi erat dengan penyakit, seperti mutasi pada gen BRCA1\/BRCA2 yang meningkatkan risiko kanker payudara dan ovarium (Collins &amp; Varmus, 2010). Saat ini, teknologi sekuensing generasi berikutnya (next-generation sequencing\/NGS) memungkinkan analisis genom pasien hanya dalam hitungan jam, dengan biaya yang semakin terjangkau.<\/span><\/p>\n<p><b>Biomarker dan Diagnostik Molekuler<\/b><b><br \/>\n<\/b><span style=\"font-weight: 400;\">Precision medicine mengandalkan <\/span>biomarker<span style=\"font-weight: 400;\">\u2014molekul indikator seperti DNA, RNA, protein, atau metabolit\u2014untuk memandu diagnosis. Pendekatan multi-omics (genomik, transkriptomik, proteomik, dan metabolomik) kini semakin dipakai untuk mengidentifikasi biomarker kompleks, seperti eksosom tumor dalam darah yang menjadi dasar <\/span>liquid biopsy<span style=\"font-weight: 400;\"> (Crowley et al., 2013).<\/span><\/p>\n<p><b>Integrasi Data dan Tantangan<\/b><b><br \/>\n<\/b><span style=\"font-weight: 400;\">Meskipun menjanjikan, precision medicine menghadapi tantangan teknis dan etis. <\/span>Big data<span style=\"font-weight: 400;\"> dari genomik dan sumber lain memerlukan algoritma kecerdasan buatan (AI) untuk analisis. Misalnya, machine learning telah digunakan untuk memprediksi resistensi obat berdasarkan pola mutasi (Topol, 2019). Di sisi lain, isu privasi data genetik dan kesenjangan akses teknologi di negara berkembang masih menjadi hambatan (Manolio et al., 2020).<\/span><\/p>\n<p><b>Studi Kasus: Dari Laboratorium ke Klinik<\/b><\/p>\n<ol>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Kanker<\/b><span style=\"font-weight: 400;\">: Terapi inhibitor EGFR (misalnya, gefitinib) menunjukkan keberhasilan tinggi pada pasien NSCLC dengan mutasi EGFR spesifik, mengurangi efek samping dibanding kemoterapi konvensional (Mok et al., 2009).<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Farmakogenomik<\/b><span style=\"font-weight: 400;\">: Variasi gen CYP2C19 memengaruhi metabolisme clopidogrel (obat antiplatelet).
